Program Description:

FAMIS is Virginia's health insurance program for children under age 19. It provides access to quality health services for children of working families. As parents, you want to provide the best for your children - but the high cost of health insurance today is keeping many families, including working families, from providing their children with the important medical care they need. FAMIS can help!

FAMIS covers all the medical services growing children need to avoid getting sick, plus the services that will help them make a speedy recovery if they do become ill or get hurt.Services covered by FAMIS include: Doctor visits, Well-baby checkups, Hospital visits, Vaccinations, Prescription medicine, Tests and x-rays, Dental care, Emergency care, Vision care and Mental Health benefits.

There are no enrollment costs or monthly premiums for FAMIS. When your children get care there may be a small fee called a co-pay for some services. Most co-payments are just $2 or $5 - FAMIS covers the rest. You do not pay for well-baby or well-child checkups.

 

General Program Requirements:

In order to qualify for this benefit program, you must be a resident of the Commonwealth of Virginia, under 19 years of age, not covered by health insurance (including Medicaid), a US national, citizen, legal alien, or permanent resident, and you must have an annual household income before taxes of less than $29,140 if two people live in the household; $36,620 if three people live in the household; $44,100 if four people live in the household; $51,580 if five people live in the household; $59,060 if six people live in the household; $66,540 if seven people live in the household; $74,020 if eight people live in the household; and $81,496 if more than eight people live in the household. For larger households, add $7,480 for each additional person in the home. Depending on your income level, you may have to pay a premium for coverage. Application Process:

Applying for FAMIS is quick and easy. Families may either:
1) Call the FAMIS Central Processing Unit to apply right over the phone, or
2) Apply at their local Department of Social Services

Families wishing to apply at the FAMIS Central Processing Unit can do so by dialing the toll free number: (866) 873-2647

Mailed or faxed applications are also acceptable. Faxed applications can be sent to 1-888-221-9402. Mailed applications can be sent to P.O. Box 1820, Richmond, VA 23218-1820.
